GET THE LOOK UP provides eyelash extensions and related products. It should be noted that the use of these products requires specialist knowledge and training, and should only be used by professionals with the correct certification.

Particular note should be made that applying extensions to your own lashes is both difficult and carries risks of discomfort and even injury. I authorize Chalette Dallas of Get The Look Up to provide and apply semi-permanent eyelash extensions to my eyelashes. In order to minimize risk of eye injury I understand that it
is my responsibility to lie still with closed eyes for the entire eyelash extension procedure or until otherwise directed. I have been fully informed regarding the methods used in the application of semi-permanent eyelash extensions. I understand as part of the procedure eye irritation, eye pain, eye itching, discomfort and in rare cases eye infection may occur I understand these risks and potential complications, and knowingly and voluntarily consent to the application and/ or the removal of semi-permanent eyelash extensions.  I agree to indemnify, defend and hold harmless Get The Look Up , its officers, directors, employees, agents, licensors, suppliers and any third party information providers to the Service from and against all losses, expenses, damages and costs, including reasonable attorneys' fees, resulting from any violation of this Agreement (including negligent or wrongful conduct) by you or any other person accessing the Service.

As the consumer of our products and services, you are agreeing that:

- The products will be used only by appropriately trained professionals

- The products will be kept, cared for and used in an appropriate manner and in accordance with any product instructions. These include, but are not limited to – temperature control or the product, use by date notified on the product, client sensitivity testing as appropriate and any product specific instructions

- In the event of accidental contact between the eyes and any adhesive or adhesive remover, the client’s eyes will be irrigated with an appropriate eyewash solution and that the client will be advised to seek immediate medical attention.

- In the event of any allergic reaction (which should be tested for before any procedure is carried out), the client will be advised to seek immediate medical attention

Complaints

Complaints must be declared directly to GET THE LOOK UP within 3 days after services have been provided.  All complaints should be sent via email to: EYELASHES@GETTHELOOKUP.COM or by phone at 305-498-4740
